Direct Hair Implantation (DHI) is an advanced evolution of the FUE technique, designed to offer maximum density, precision, and faster healing. The core difference lies in how the hair follicles are implanted into the recipient area.
In standard methods, the surgeon must first open microscopic channels (slits) in the scalp and then insert the hair grafts into those channels using forceps in a two-step process. DHI combines these two steps into one seamless action using a specialized medical tool called the Choi Implanter Pen.
The Choi Pen features a micro-needle that is loaded with a harvested hair follicle. As the specialist gently pierces the scalp with the needle, the graft is directly implanted into the skin at the exact same millisecond. This one-step process protects the fragile hair follicles from external damage, ensures they spend minimal time outside your body, and allows for incredibly precise placement.
For many patients, one of the biggest hurdles to getting a hair transplant is the thought of shaving their head. It can be difficult to take weeks off work or explain the sudden change in appearance to friends and colleagues. This is where the DHI technique shines as the ultimate solution for privacy.
Because DHI utilizes the Choi Implanter Pen to insert follicles directly between existing hairs, we do not need to shave your entire head. We only shave a small, strategic “window” in the donor area at the back of your head, which can easily be covered by your surrounding longer hair.
The recipient area where the hair is thinning remains completely unshaven. This means you can return to your social and professional life almost immediately, with virtually no one noticing that you have undergone a hair restoration procedure.
If you are experiencing thinning in specific areas rather than complete baldness, DHI is often the superior choice. When inserting grafts between your existing, healthy hairs, standard FUE methods carry a risk of damaging the neighboring healthy roots during the channel-opening phase.
The microscopic precision of the Choi Pen eliminates this risk. Our specialists can carefully navigate around your existing native hair, implanting new grafts in the exact spots where density is lacking.
Furthermore, because the pen allows us to place grafts much closer together, we can achieve an exceptionally high density (grafts per square centimeter). This results in a much thicker, fuller, and more natural look that seamlessly blends with your surrounding hair.
We Choose Medical Integrity Over Marketing Hype.
Many clinics market DHI as a flawless technique that is superior to FUE in every single scenario just to charge higher prices. At Fair Hair Clinic, we believe in telling you the honest medical truth.
Graft Limits: DHI is incredibly precise, but because each graft is loaded into the Choi Pen manually, the procedure takes significantly longer. Therefore, DHI is not ideal for massive baldness cases requiring over 3,500–4,000 grafts in a single session.
Technical Difficulty: DHI requires an exceptionally high level of skill and experience. If performed by an inexperienced team, the Choi Pen can damage the hair follicles.
The Verdict: If you need localized density or want a shave-free procedure, DHI is unmatched. If you need maximum coverage over a large bald area, a combination of FUE and DHI, or FUE alone, may actually yield a better, healthier result. We will evaluate your hair honestly and recommend what you actually need.
Because DHI bypasses the traditional channel-opening phase, the physical trauma to your scalp is significantly reduced. Standard methods create larger incisions to insert the grafts, which leads to more bleeding, longer scabbing periods, and more post-operative swelling.
With DHI, the Choi needle makes a microscopic puncture that is immediately filled by the hair follicle itself. This acts like a natural seal, drastically reducing bleeding and tissue damage.
As a result, post-op swelling is minimal to non-existent, scabs are much smaller and heal much faster, and the overall redness of the scalp disappears in a fraction of the time compared to other methods. Most of our DHI patients report feeling completely comfortable and looking great within just 5 to 7 days.
We want you to feel completely relaxed and prepared for your treatment day. Knowing exactly what happens during the session can help ease any pre-surgery anxiety. Here is a realistic look at your DHI day:
Preparation & Hairline Design (Hour 1): We clean your scalp, apply initial local anesthesia, and work with you to draw and finalize your customized hairline design.
Graft Extraction (Hours 2–4): Our senior specialists extract healthy hair follicles from your donor area using microscopic precision tools.
Lunch Break: You will enjoy a comfortable break and a nutritious lunch to keep your energy up.
Implantation via Choi Pen (Hours 4–8): The medical team carefully loads your grafts into the Choi Pens and implants them directly into the target areas. You can relax, watch TV, or sleep during this phase.
Post-Op Care Briefing: We clean the area, apply protective dressings, and explain how to use your personalized aftercare kit.
